VINTAGE Gap Clothing Company Store Organizer Planner Used.  There are some white spot stains (see 2nd to last picture).  The Gap label is creased and has wear (see last picture).  Measures about 8 1/4 inches tall and 6 1/4 inches wide.  It includes 2 pencils, an eraser, and a pencil sharpener.  From a smoke free home/no odors.
