An original Civil War document dated 1863. This document says Moses Chapman of the 12th Regt N.H. Vols has been found to be suffering from chronic diarrhea and is unfit for duty and a furlough is recommended. It is signed by a Surgeon. The document has some wear and some stain marks, there are some areas on the edges that are worn off.
