  I have searched high and low for the model number on the front of this box.  I found one that goes with the "EAF009" like is on this box, and it is the following:

This pentagonal filter is in its original plastic wrap and measures 12.64"L x 8.03"W x 2.64"H.
The filter is still-sealed within its original plastic, but the outer box shows some wear.
