This is a Genuine OEM Mazda Passenger side non-adaptive (non-AFS) LED Headlight; the part number on the assembly is DGJ1-51-030.  ALL mounting tabs are 100% intact---not "repaired", but totally intact;  there is one guide pin that is cracked, but it is still attached and can be glued (or even removed as does not affect the mounting) this is shown in the 17th photo. 

This assembly comes COMPLETE with the bulbs and ballast as shown.  As you can see in the photos which I took outside in the sun, that although this is not new or perfect, the lens looks GREAT with only the most minimal of wear.  REST ASSURED this is a GENUINE OEM headlamp and you will have ZERO fitment issues that often occurs with aftermarket reproduction assemblies.  Aftermarket assemblies are much cheaper because they use inferior components-from thinner wiring, lighter housings, poorer quality LEDs (the main beam AND the Daytime running LEDs) and importantly, right down to a cheaper, thinner lens which will yellow and age much more quickly than a genuine KIA lens. I think it fits all the listed models, but PLEASE compare this lamp's photos with your original;  a link to a Genuine Mazda Parts website is below so you can double-check fitment and make sure this is the assembly you need is below:
