This 4.5" x 7.5" booklet has 18 pages and is in very good condition. The ID card has been torn out using the perforations. I believe it is only a card naming the carrier as taking Antabuse. The 2nd half of this book lists dozens and dozens of foods, other drugs, and supplements that contain alcohol. They are listed so the user does not accidentally using these items as a reaction may occur. 

Otherwise, the first half talks about the drug itself and after a few pages goes into a text on the hidden alcohol in other products.
