Used in an office for a while, and not needed anymore.  Great condition, and no problems.

Power adapter not included - phone only.  Works with POE from an adapter or POE enabled switch, or you can order 5V 2A adapter for $10 - $20 on eBay, etc.

I can send actual picture if you want, but there are several, so don't know which one you will get until ordered.  I have looked them all over, and see no visible physical problems with any of them.
