Obverse: above there is the Small Coat of Arms of Ukraine with the semicircular inscription НАЦІОНАЛЬНИЙ БАНК УКРАЇНИ (National Bank of Ukraine); in the coin centre, within the circle, there is depicted a fragment of the synagogue building facade, to the right and left whereof conventionalized ornamental patterns are; the coin issue year indication 2012 is to the right; below there is the legend: П`ЯТЬ ГРИВЕНЬ (five hryvnias) on the albata coin; the logo of the National Bank of Ukraine Mint is to the left of the coin centre.

Reverse: against the background of the conventionalized decorative building plan there is depicted the synagogue; above there is the inscription СИНАГОГА в м. ЖОВКВА (synagogue in the city of Zhovkva).
