My grading is as follows.  Anything below a VG+ is used for rarer records.  I do not list common records below VG+.

M - I've rarely seen this level, even on sealed records.

NM - Possibly a few light ticks, mostly at lead in, still a quiet background.

EX (also known as VG++) - A few ticks or pops, especially at lead in.  

VG+ - More than a few ticks or pops per side.  The noise doesn't interfere with the music. 

VG - The background is no longer quiet.  The music still overpowers the noise.

VG Minus - Not junk, but the noise is sometimes intrusive.

G+ - Noise is often intrusive, but the music is usually enjoyable.

G - Only sometimes is the music enjoyable.

F (Fair) - Noisy.

P (poor) - Your stylus is at risk.
