Vintage 1972 Uneeda Co. Brooklyn, N.Y.; Miss Deb Doll & Box Hair That Grows and Mod Mini Skirt Outfit #9617.  The doll is in near mint condition and the box is very good.  The hair has not been combed out; an elastic cord is coming out at top of the head, but I'm not sure if the hair function is working properly (didn't want to test it).  There is no evidence that it has ever been played with.
