There are several Elrathia kingii, on this plate which were found in the Wheeler Formation (part of the House Range) in Utah, U.S.A.  There is at least one intact trilobite, two molt ,, several impressions and many partial bits.

 The entire plate measures 3.88 by 4.3 by 0.77 inches.
