Vintage silver prayer box amulet necklace from the Middle East.  Pre 1968.  This interesting double pendant is a prayer box.  One end cap is missing so you can see that there is something inside.  I have not tried to remove it.  It seems to be made of a natural material, perhaps paper or wood.  See detail photo.  This type of amulet often holds a copy of the Quran (Koran).  The center drop below the box has a turquoise and a coral bead.  It appears that one of the drops is missing.  The upper round pendant has a large loop on the back as though it could be worn by itself on a thick cord with the three loops hanging down.  This artifact was just as you see it when purchased, including the "schmutz"  you can see in the photos.  I have not tried to clean it, though I did scrape a bit off with my fingernail.  
My Great Aunt purchased this in the Middle East at an open air market in 1968.  She proudly displayed it in a glass topped box for many many years.  
The box portion is 2.75" wide by 2.5" tall by .5" thick.   The center drop with turquoise and coral is 2" long, the other five are 2" long.  The round medallion is 1.5" diameter.   The length of the chain is 11" from the back of the neck to the top of the box.
