One tile from Dungeons and Dragons Geomorphs Basic Dungeon Set.  This was from the compliation of geomorphs that TSR published in 1980 (they were previously published in three sets).  These were square tiles with dungeon maps printed on them that could be assembled in different combinations, fitting together at the edges.  Uncut, the squares form an 8 1/2 by 11 inch rectangle; this one is an individual cut square tile, roughly 5 inches in size.    

Condition fair to good; note the numbers written in pencil in the rooms, as well as the discoloration and small stains in places, and note that the tile is folded in half.  The edges are worn, and were cut out by hand with a scissors.  Note that the image is an actual scan of the item.
