This beading book is called Pipe Beads: 26 Beaded Projects. The designs were created by Steve Hoffman. It was published by Design Originals in 1994 in the US. The patterns are as follows:  Key Chain, Box Braid Key Chain, Single Strand Bracelet, Three Strand Bracelet, Two Strand Bracelet, Medicine Bag, Medicine Bag with Fringe, Hematite Necklace, Adjustable Hat Band, Bone Pipe & Red Bead Earrings, Long Sleeve Shirt, Breastplate Choker, Brass & Turquoise Tassel Necklace, Teal & Black Earrings, Man's Breastplate, Woman's Breastplate, Turquoise, Black & Silver Necklace, Coral & Blue Necklace with Concho, Dream Web, Dream Catcher Necklace, Watch, Brown & Bone Pipe Bead Choker, Skull, and Necklace with center Carved Bead. Each pattern gives you all the supplies needed to make the project. It has very clear worded instructions on how to make each one.
