Antique Opium Drug Store Pharmacy Doctor DEA History Ellis Glasgow KY Kentucky

A great original Treasury Department issued order form for Opium or Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations! This 1923 issue (the red duplicate) was recently shown on the History Channel during a program on illegal drugs. Please see photos!



It is the only issue with the directions printed on the back.



This form is ornate with a beautiful poppy graphic on the top corner. It was issued to L. C. Ellis Drug Co., in Glasgow, KY. An order for Papine (containing Morphine) was placed on August 21, 1929. This form was kept by the doctor/pharmacist for his/her records, the top original was sent to McKesson Peter Neat and Rich Company, in Louisville, KY to be filled.




The form is large, 10 3/4" by 8 1/2", is watermarked, "USIR", has light bends, otherwise, condition is excellent...a beautiful document!

 This form is ornate with a beautiful poppy graphic on the top corner. It was issued to L. (Luther) C. Ellis Drug Co., of Glasgow, KY. An order for Pantopon was placed on January 30, 1932.  This form was kept by the doctor/pharmacist for his/her records.

 
The top original was sent to Geo. J. Ellis, also in Glasgow, KY, to be filled.



The form is large, 10 3/4" by 8 1/2", is watermarked, "USIR", has a corner bend, and a few carbon rubs, otherwise, condition is excellent...a beautiful document!
