USED, CLEAN, DURABLE, VERY GOOD CONDITION! CLEAR plastic "DAIRY DOOR".   PART # 69918-2.  THIS IS CLEAR/SEE-THROUGH AND LIFTS/ROTATES UP.  EACH side has a small cylinder-shaped knob that allows this to fit in the refrigerator door.  it's meant to go at the very top.  you will get the exact item shown.  please see the pictures with the tape measure for measurements.    the item does have some normal wear, light scratches, etc. 

this comes from a MAYTAG side-by-side REFRIGERATOR model # RSW24E0DAE.  (but i think the part may work with other models).   this particular fridge was bought new in about 1994 and was very well taken care of (and just barely used at all in the last 10 years).   the last picture comes from a parts diagram i found online - if you go to the sears website it says that most of the parts from this model are unavailable.
