Measurements are done with the item laying flat. Measurements for waist, hip, chest, or bottom of hem are doubled for ease when comparing to actual body measurements. I recommend comparing garment measurements to items that currently fit you well to help estimate fit as sizes can vary quite a bit, especially on older, pre-owned or vintage items.
