It comes as per the picture with a custom disk label and cover.

Overview
Music Mouse turns the ST into a musical instrument with complete control in the hands of the performer.
It takes mouse movements inside a grid on the screen and transforms them into four moving voices that can be assigned to different MIDI channels and sounds depending on what you do on the keyboard. Control faders, tempo, transposition and host of other features.

As mentioned the game has been tested on an Atari 520 STFM. As much as I love playing these games, and believe me I do lose a lot of time sometimes when testing games, I can only test them up to the point of the gameplay starting. There is no way that I have the time or the ability to play every game through to the end.
With this in mind and due to the medium that they are on I cannot guarantee that the disk will not have any errors further into the game. All of these games are well over 20 years old so there is a chance that a game could fail at some point, regardless of where it was purchased from.
When purchasing this game please keep in mind the model and specifications of your Atari.
