This is a 2005 Ohio State Legends of the Scarlet and Gray Collectible coin/medallion.

These were a limited edition. There is a Wendy's logo on the card, they may have been sold there. There were 12 total in the set.

The coin is about 1.5 inches wide.

It comes in a thin plastic holder. The coin can be removed. The back of the holder card has some player information. 

The front of the coin shows the player, with their name in the border.

The back of the coin says O Legends of the Scarlet & Gray.
