Radio Shack A/V Control Center  Model 15-1987  Audio, Video,  Optical & S-Video. Tested and in good working condition. Does not come with a power cord. I had to use my own personal one to test it. Just requires a 12V power cord and they are pretty cheap online. Minor scratches on it otherwise its in decent looking condition. Shipped with USPS. Thanks for looking.
