Held on September 25, 1968, this event honored Chicago Bears owner George Halas. Listed on the interior as the ceremony's "Speaker," Lombardi has applied a potent signature on the front cover of this crisp program.

Program, itself, is a tri-fold and measures 9 1/2" x 20" when fully extended.
