Price is per individual card. Conditions vary slightly, but all are lightly played to near mint. I however am not a professional grader, so your mileage may vary. May show light wear on sides or faces of cards. Picture is good representative of overall quality of cards, but the card you receive may not be pictured. Every card will come in individual sleeve.
