It is made by Universal Player Piano Roll Company and is #91265 Follies of 1909 with 5 selections. It has BOTH of its Spool Ends as well as it's Original Tab and paper label. It shows wear on the Tab area as well as minor edge wear on one side of the leader. However it ALL clears up before the notes start. The selections are as follows:
