A genuine old Topps baseball card!

I use the most conservative combination of the 2004 Standard Catalog of Baseball Cards, and the PSA website from around that time, as follows:  (Ignore the eBay condition indicator above)

Excellent-Mint (Ex-Mt) - the highest grade I use.  Perfect, except may have some visible surface wear or a printing defect that does not detract from its overall appeal, may have a very light scratch which may be detected only upon close inspection, corners may have slightly graduated fraying, may have minor wax stains on reverse or some off-whiteness on borders, centering may be as far off as 80/20 on both sides.

Excellent (Ex) - Corners fairly sharp - may have minor chipping on edges or very minor gum, wax, or product stains.  Centering must be 80/20 or better on both sides.

Very Good (VG) - Corners may be slightly rounded - surface wear noticeable but modest - card may have a minor crease or a few very minor creases - may have slight gum stains in front and major gum stains on back.  Borders may be yellowed or discolored.  Centering must be 90/10 or better on both sides.

Good (G) - May have several creases but not so heavy as to break the surface of the cardboard, and considerable discoloration, but no intentional damage or abuse.

Fair (F) - May have thumb tack holes in or near margins, evidence of having been taped or pasted, small tears around the edges, minor pen or pencil writing on backs, or be missing small bits of paper, but basically a complete card.

Poor (P) - Any of the following automatically puts a card in this category:  areas torn off, writing on front, holes from a paper punch, major portions of the design missing, other defacements.
