3.Will Not Fit on Rear Hub - Your factory axle part may be sticking out from the hub bore, the hubcentric spacer with wheel centric lip will not fit if the axle part sticks out exceeded 50mm.

2. Make sure you get 5-7 FULL TURNS of the lug bolts before tightening.
