This is circular memorial plaque made of concrete. It features a paw print in the center with a wreath of leaves surrounding it. The inscription "Gone But Not Forgotten" is repeated around the edge of the plaque. The plaque measures approximately 5 inches in diameter and weighs 0.74 pounds. It is in good condition. There is a small piece of tape on the back of the plaque.
