Marked “USE WITH 952016 RF PROBE ONLY”. This is a precision accessory for Boonton RF probe systems, not a general-purpose inline attenuator.
Connector configuration is as shown: inline coax body with tee adapter. RF connectors are N (F) type; probe interface is specific to the Boonton system.

Condition is clean. I measured 10.99 K from HV terminal to ground, 10.88K from HV center terminal to N-connector center terminal, 1.015 meg from center terminal of N connector to ground, and .2 ohms between the two N connectors., I have not tested this with a compatible Boonton probe, so selling as untested.
