This is "Andy Warhol's Index (Book)" by Andy Warhol, in a stated first edition published by Random House in 1967. This edition features reflective black and silver card covers, and many inserts, as follows:
(1) double spread color castle pop-up, with text "We're Attacked Constantly" (2) red accordion fold-out (3) color bi-plane pop-up (4) spring-loaded Chelsea Girls disk (5) hairy-chested dodecahedron sculpture, intact, but separated from its attaching string (6) MISSING: Lou Reed 45rpm flexi-disc.(7) folding poster with dual nose overlays (8) Hunt's Tomato Paste pop-up (9) sheet with eight moisture-sensitive tabs; only one is present.(10) Latex balloon attached between two pages, which have stuck together as the balloon deteriorated over the years, 
A iconic book of the PopArt movement and the primary production of Warhol's Factory. Condition of this book is Very Good, showing moderate ruubbing to the covers and some early pages beginning to separate from the binding, but still holding.
