This is a surplus/Engineering stock II-VI Incorporated CO2 Laser  Scan Lens for precision Laser machining. Part was not extracted from field equipment but was sitting on shelves for most of its life time.

Specs indicate that this lens was designed for a small spot size (more energy density) on a narrow field for high precision machining. It was meant to be mounted on a system with compound beam positioning where laser scanner and linear stages are used. (Scan positioners for fast moves, then linear stage for area to area moves)
