Please include what you would like engraved on the ID below and cut and paste in comments to seller or in eBay messaging. Please verify all spelling as we are not responsible for misspelled request. ( we engrave exactly as it is sent to us).

We can engrave 6 lines (20 Characters Max. in each line) on the back side and 4 lines (13 Characters Max. in each line) on the front side (BRACELET ONLY) of personal or medical information on the ID plate. 
If you would like to engrave just backside, leave front side fields empty.

RETURN POLICY

Unfortunately, we do not accept returns of Medical IDs that have been custom engraved.  The custom engraving personalizes the Medical ID to your unique health information and cannot be re-used.  To return a Medical ID that has not been custom engraved, please contact us via Ebay to obtain a Return Authorization Number.

WARRANTY POLICY

Our goal is to make you a customer for life.  We never want you to worry about your Medical ID.  If at any time you find your Medical ID to be defective in materials or workmanship, please contact us immediately.  We will repair or replace your Medical ID at our discretion.  This warranty does not cover the following: 

·       Normal wear such as scratching, dents, or tarnishing

·       Accidental damage 

·       Repairs or modifications by anyone other than Med ID Pro
