Exact item shown, Anaren hybrid couplers.  The last couple of photos are the spec sheet for the surface mount hybrid part that is inside the connectorized case.  you will need to supply your own terminator for the "iso" port, it's a standard 50 ohm SMA terminator.  You are buying one at a time, choose the quantity you want from the drop down list.  One of these has some deep scrapes on one side of the aluminum case, and another has the 0 deg connector at an angle, but they are in the "as tested" condition so clearly they have survived this abuse.

I used a dual trace analyzer because it lets you see how well the two channels track - you can see that they are very close.  The dual trace analyzer plots show both the through parameter (S12) and also phase.  My dual trace analyzer only goes to 3 GHz, so to check the top end of the range I checked one of these on a 20 GHz analyzer, that's the last pair of screen captures (one for the 0 deg port, one for the 90 deg port)

Measured Data: 

• S21 per output ≈ −3.25 to −3.35 dB @ 2 GHz (20 GHz VNA) << remember the splitting uses 3dB, so it's loss above 3dB that matters, so the actual insertion loss is .25 to .35 dBc,
• amplitude balance |ΔA| ≈ 0.30–0.35 dB (0.5–2 GHz), 
• phase ≈ 90° nominal (plots included)
