This is the black driver-side power seat switch bank with the separate lumbar toggle — main slider, recline control, and the lumbar switch in one assembly, mounted on the outboard side of the driver's seat. The same assembly also came in gray, so match your interior color from the photos.

The lumbar toggle is the whole distinction here: the base switch without it looks nearly identical, shares the same part number family, and will not restore lumbar control on a lumbar-equipped seat. Count the switches on your original against the photos — that is the check that matters. These fail the way seat switches fail: one function stops responding while the others keep working, or the seat only moves when you wiggle the knob — and it gets misdiagnosed as a dead seat motor when it is a worn switch contact.
Everything in the photos ships: the complete switch assembly with the mounting plate and connector. Seat trim and wiring harness are not included.
