Sergeant Major Polyakh has been participating in the Patriotic War since August 1942.

He took part in the defense of Stalingrad.

During the street fighting for the city of Tarnopol from April 1 to April 3, 1944—despite a severe blizzard and the icing over of his mortar and ammunition, and while under enemy machine-gun and artillery fire—his mortar operated flawlessly, day and night. Through his mortar fire, he destroyed 6 machine-gun emplacements and up to 30 enemy soldiers.
