Provide your prescription (RX) by EITHER:
- Filling out the digital RX form below and including it in your order notes, OR
- Sending clear photos of your paper prescription via eBay Messages.
2 Confirm Your Frames & Complete Payment
Tell us your chosen Frame (Item title or number from our store).
We will create and send you a combined invoice (Frame + Your Selected Lenses) for easy checkout.
Prescription (RX) Form & Terminology Guide
To ensure perfect vision, please provide all details from your prescription. Here's what each term means:
| Abbreviation | Stands For | What It Means |

| OD | Oculus Dexter | Your Right Eye measurements. |
| OS | Oculus Sinister | Your Left Eye measurements. |
