This is Ice Climber for the Nintendo Famicom Disk System.
The FDS version was released in Japan on November 18, 1988 as a Disk Writer-exclusive title, distributed through Nintendo's Famicom Disk Writer rewriting service at toy stores and other retail locations.
Because it was a Disk Writer-exclusive release, it was not originally sold as a conventional packaged retail FDS game.

For identification purposes, a replacement/non-original label has been attached to the disk, as shown in the photos.
