This is one of the very few was decorations awarded to American Militiamen during the American revolution war. 

Obverse- Heart under heart with word “Fidelity” inside wreath. 

Reverse- Word “Amor” at the top and words “Patrice Vincit” on the bottom 

Inside the wreath is hand engraved “For Merit” 1778

Oval 2” X 1 1/2” pewter 

I do not believe the ribbon is original to the medal
