It includes the Slide duplicator itself, the original box, and the cardboard holders inside the box that keep it secure. It also includes a Kodak Ektachrome slide as a little bonus. It does not include the ring attachment or the instruction manual.

Item is untested however I see no issues that would interfere with it working properly. The bellows appear fully in tact, letting no light through.
