The card in the scan is the card you will receive.  I combine shipping in the US.

If you are outside of the US and want multiple items, DO NOT purchase each individually.  Instead, add them to your cart and then do a checkout/payment for all of them in one step.   I can not combine global shipping packages once they are purchased individually.
