Gemeinhardt model 4PMH intermediate piccolo with hard case and outer leather (?) style case. I was able to get it to play a few notes, but it will definitely need some TLC to be in fully playable/functioning condition, including but not limited to a deep cleaning, new pads, and spring/key adjustments. Resin body appears to be solid at least from the outside.
