Approximate dimensions are approximately 12" long & 2" at the widest point.   The Locker Tag is authenticated as 'Team Issued' and was input by the on-site MLB authenticators in SF, who apply the holograms and enter the details of the item.  Authentications can be verified through the MLB.com site.  This name-plate carries the hologram YP096588.  I have attached a screen shot for your review of the MLB authentication serial number search result.
