These were pulled from Mergepoint switches. The front mounted board has the virtual media USB port & LED indicators. The board will include the data ribbon cable, as pictured. These will be perfect to replace damaged USB ports on the Mergepoint switches listed below:

They will also fit some Sun Mergepoint KVM switches made by Avocent, with the same model numbers as above.
