Password Journal from Girl Tech.  Includes instructions sheet.  I don't have the notebook or pen (wrote in invisible ink and you could see it with the light).  Light works, voice works, asks for you to record your Password, says hello and goodbye.  Seems to work fine.  Sold as found.
