You are looking at an older practical unused adapter called the LT-6 made by Astatic during the 1950's to 1970's. This Line Matching Transformer was used to get your mic and audio equipment in sync to get the right impedance depending on the mic you were using and what type of audio equipment.
It will match the impedance of your microphone to your audio equipment so you won't have any issues.
Pic # 2 is from the old Astatic advertisement showing the models specs and uses.
As you can see the item has typical lite wear associated for it's age and works perfectly.
The LT-6 needs the Amphenol 4 pin cable connector as well as the 5/8 X 27 Amphenol connector.
I showed the connectors you need to make this transformer workable in pics # 7 - 9.
