The letter is 200 years old and condition reflects. It has several tears and is fragile.

It has been folded twice.

I believe it was sent from Tuscany to Sardinia (Genova). The state of Sardinia included Piedmont I believe.

You would need to be able to read the actual letter for clues. I don't know if it is actually in Italian, or a more ancient dialect.
