"A perusal of the literature which bears on the question of the treatment of acute rheumatism (rheumatic fever) is a task from which few would rise with any definite idea as to how that disease is best treated."

This opening sentence of RHEUMATISM: ITS NATURE, ITS PATHOLOGY, AND ITS SUCCESSFUL TREATMENT (1881) was quoted from the important 1876 paper in which Thomas John Maclagan (1838-1903) introduced salicylates as a remedy for acute rheumatism. Before becoming a London practitioner sought after by a glittering circle, Maclagan conducted his investigations in settings that were far from glamorous.

Maclagan himself had recovered from typhus fever.  Working at the Dundee Royal Infirmary, he gained experience with fever patients. He knew that overworked staff members, including his predecessor, had died of typhus fever. Maclagan insisted on a better diet and good ventilation for patients and also introduced use of the thermometer, then considered by many a toy

Maclagan is best known, however, for his trial of the glucoside salicin in acute cases of articular rheumatism. As shown in the Notes from the Editors accompanying this facsimile of RHEUMATISM, the investigations that later resulted in the use of aspirin and today's NSAIDS can be traced to this outstanding Victorian physician."
