Includes game, case, and manual. The actual game disc is in flawless condition with no scratches. The jewel case plastic does have some scratches, scuffs, and marks. I also noticed that the artwork appears to be warped/bent. There is also some discoloration/stains on the inside of the artwork as shown in the pictures. The manual is also pretty bent as well.
