Disk is in excellent condition. 

Comes with the English and foreign language manuals.  The English one has some bending and slight creasing on the right side from how it was stored.  The Foreign one is in excellent condition.

Case and cover art are in very good condition, case does have the name 'Jon'  written on it, please see pictures.
