This $5 note is a Type 2 Brown Seal note which has an extra set of brown bank charter numbers, in this case 13689. During the Great Depression, banks would hold deposits such as coinage or treasuries as securities in exchange for these notes printed with the National signatures on top and the bank signatures at the bottom. This bank was originally known as Canal Bank in New Orleans. After the bank holiday it re-emerged an The National Bank of Commerce and then later First National Bank of Commerce, then it was purchased by Bank One and later by Chase, thus it is now part of Chase.
This note is in average condition, though quite collectible.
