This listing is for a Sloan Optima exposed water closet cover plate with 4 screws.  Has chrome plate finish.  Model # for cover plate is EL201 and product code is 0305219.  Model number for the flat head cap screws is EL152 CP.    Items are new in the packages.  See pictures for details.  Below is a description of the model EL-201 from the Sloan company website:
