The purpose of this medal is the 500th Anniversary of the Sacrament of the Madonna of Sasso in 1480.

Front is a depiction of a monk behind the Virgin May with a Cathederal

 and Cross as well. It says:

The back says:

V CENTANARIO dell' Erems Sacro Monte e Santario della Madonna del Sasso 1480*1980
