Manufacturer is not marked.  Matches pattern of Bryant, Hubbell, GEM, Circle F, Miler and others that offered this pattern.

Scuffs Oxidation and Shelf Wear.  No Cracks or corner chips.  Receptacle openings have molding flash around them proving this was never installed -see photos.
